A request to change your name with the University of Michigan-Dearborn must be made in writing to the Office of the Registrar.

If your legal name has changed since you last attended UM-Dearborn, please complete a Name Change Form. Residency
Read Carefully: The information provided in this section will not affect your eligibility for admission, but it will help determine your tuition rate. If you believe you qualify for in-state tuition and any of the following circumstances apply, you are required to file an Application for In-State Tuition with the Residency Classification Office and be approved before your status can be changed to in-state.

For questions or assistance related to in-state tuition classification, please contact the Residency Classification Office on the Ann Arbor campus at (734) 764-1400. There are established deadline dates for submitting an Application for Resident Classification and it is recommended that applications be submitted as soon as possible.

Note: The Elementary, Secondary, Children and Families, HHS, and Sports Management majors require the selection of one or more concentrations. Please refer to the CEHHS Areas of Study page to learn about and select your option(s). Enter your selection here. If your major does not require a concentration, please enter “no concentration”.
The following CECS majors require a concentration: Computer and Information Science, Cybersecurity and Information Assurance, and Human-Centered Engineering Design. If choosing one of these majors, please include your selected concentration. Available concentrations can be found in the Undergraduate Catalog.
If you majoring in Integrative Studies, please provide your three concentrations.
Previous College Information

Starting with the current or most recent, list ALL post-secondary schools you have attended since last attending UM-Dearborn. Include all colleges or universities (foreign or domestic), as well as vocational or military training, dual enrollment, or other non-degree educational programs. 

If your institution is not found below, please select "Unknown College (Dearborn, MI)" and your institution will be added once transcripts have been received. 

Please request that your official transcripts be sent directly to your new readmitting college.
