Group Visit Request Form

Thank you for your interest in scheduling a visit to the University of Michigan-Dearborn! We are excited to work with you to plan your visit. Please use this form to submit your group visit request for review by our Events team. 

We will confirm the details of your request within one week and if we're unable to accommodate your preferred date we will work with you to find an alternative.

*Please note that we require at least 2 weeks' notice in order to ensure appropriate staffing and space for reservations.*

Acknowledgement to Agreement and Waiver

PARTICIPATING ORGANIZATION AGREEMENT AND WAIVER: 
Participating Organization understands and acknowledges that participation in the Program is voluntary and that as a condition of the participation of Participating Organization and its children in the Program, that Participating Organization agrees lo comply with all Program rules and requirements, including but not limited to: (1) providing written acknowledgement informing participants and their parents/legal guard and that the Participating Organization is responsible for the supervision of any student participating in the Program; (2) ensuring that children in the Participating Organization are aware of the Program's standards of conduct (http://childrenoncampus.umich.edu/wp-content/uploads/2018/04/Code-of-Conduct-042618.pdf): and (3) immediately notifying the Program Administrator of any concerns related to the health, safety, or security of Participating Organization's children, other participants, or staff.

Participating Organization agrees that adult chaperones from Participating Organization are responsible for the supervision of the children from Participating Organization during the Program, and must remain in the children's general vicinity throughout the Program. Participating Organization agrees to indemnify, defend and hold the university harmless for any and all claims, causes of action, demands and damages arising out of or relating in any way to the program activity.

This Agreement is governed by and construed under the laws of the State of Michigan without regard for principles of choice of law. Any claims, demands, or actions arising under this Agreement must be brought in a court with applicable subject matter jurisdiction sitting in the state of Michigan and Participating Organization consents to jurisdiction of a Michigan court with appropriate subject matter jurisdiction.

Authorized Representative is duly authorized to execute this agreement and waiver on behalf of the Participating Organization, and the terms and conditions of this Agreement are binding on Participating Organization, its representatives, and assigns.
